By registering for the Smart Hoops program, I, the undersigned parent or legal guardian of the child named in this registration, acknowledge and agree to the following terms:

1. Photo and Video Consent

I understand that photos and videos may be taken during Smart Hoops sessions. I grant Smart Hoops and its affiliated entities the irrevocable right to use, reproduce, publish, and distribute any images or recordings of my child or myself for promotional, marketing, or educational purposes in print, digital, and social media formats, without compensation.

2. Release of Liability

I acknowledge that participation in physical activity, including basketball-related drills and games, carries an inherent risk of injury. I, on behalf of myself and my child, voluntarily assume all risks related to participation in the Smart Hoops program, including but not limited to injury, illness, or property loss.

I agree to release, waive, and hold harmless Smart Hoops, its staff, occupational therapists, volunteers, facility partners, and affiliated organizations from any and all claims, liabilities, or demands arising from participation in this program, whether caused by negligence or otherwise.

3. Medical Emergencies

I authorize program staff to seek emergency medical treatment for my child in the event that I cannot be reached. I understand that I am responsible for any resulting medical expenses.

4. Consent to Data Collection and Storage

I understand that Smart Hoops will collect and store personal and health-related information submitted during registration, including names, contact details, medical notes, and support needs. This information will be securely stored and used solely for program operation, safety planning, and communication. Data will not be sold or shared with third parties outside of program partners or medical providers (when applicable)
